Location: Dorset – Hybrid working home and office base (Beech House, Poole)

Hours: 37 hours per week (part-time considered)

Salary: £28,672 pro rata

Contract length: Fixed term 10-month contract

The latest public health data tells us that people with a diagnosis of severe mental illness (SMI) are statistically more likely to die prematurely (before the age of 75) than adults who do not have SMI.

We know that many people with an SMI diagnosis do not attend their annual health check and we are committed to reducing this number, enabling them to access the services they need.  

You will work collaboratively with a variety of key stakeholders including:

  • Primary Care Networks (PCNs), GP practices and Physical Health Support Workers.
  • patients, family members and carers
  • the voluntary and community sector (VCS)
  • NHS Dorset colleagues

About you

This post requires someone with a person-centred approach who has well-developed communication skills that help build rapport quickly. It requires a proactive and resilient person with the ability to work confidently and independently.
