One-off Wellbeing Grants significantly reduce hospital stays by having CAN Pathway Coordinators address barriers to discharge and enable patients to return home promptly.

CASE STUDY – MR AB

Mr. AB, an 88-year-old patient, faced delayed hospital discharge due to his wife’s injury and a recent relocation, which hindered their ability to
create a suitable home environment. A bed and unpacking of numerous boxes were necessary for his safe return.

A Pathway Coordinator initiated a multi-agency approach and engaged the Red Cross Transfer of Care Hub to assess the required home
adjustments and the BCP Handyvan service for heavy lifting assistance. The local Rotary Team unpacked and moved boxes. A single bed was secured from Faithworks’ Storehouse. This collaborative effort eliminated the need for a costly hospital bed, as reassessed by the ward therapy team.

Mr. AB was discharged six days early, saving the hospital £3,000. Additionally, the coordinated community support provided a cost avoidance of £1,045 – a total saving of £4,035.
