Helping people live healthy, happy lives

Staying healthy is important to us all. It means maintaining good mental and physical health and getting practical and emotional support. CAN, and its Community Connectors, bring colleagues from across the NHS, our local councils, charities and community groups together.

The team of Community Connectors support both Acute and Community Hospitals across the BCP area, while expanding their community engagement and support to wider professionals and the public through signposting, information stations and local events.

The Virtual Hub remains a key component of our service,  as it provides frontline health and social care professionals with access to information, guidance, and support from the voluntary sector.

The CWC service is dedicated to helping residents across Bournemouth, Christchurch, and Poole (BCP) access the support they need to thrive. Whether it’s promoting physical and mental wellbeing, offering emotional support, or connecting people with local resources and activities, our initiatives are designed to make a real difference in people’s lives.
