Community Connectors work across University Hospitals Dorset (UHD) and community hospital sites in the Bournemouth, Christchurch and Poole areas. They provide additional support to hospital discharges and works with hospital teams to facilitate faster discharges and prevent frequent readmissions.
We achieve this through signposting, providing information and guidance, and connecting people to organisations, groups, and services that can help them live well in their communities. This can range from cleaning and shopping to befriending, or finding social activities.
Our mission
Our mission is to ensure a smooth and safe transition from hospital to home for all, ensuring everyone receives the information and support they need for their recovery at home and beyond.
