The Wellbeing Collaborative has a dedicated team of three Pathway Coordinators working across UHD and community hospital sites in BCP. We provide additional support to hospital dischargees and work with hospital teams to facilitate faster discharges and prevent frequent readmissions.
We do this through signposting, information and guidance, connecting people to a wide range of organisations, groups and services that can help them to live well in their communities. This can range from cleaning, shopping, befriending, social activities, local support groups, etc.
Our Mission
Our mission is to help ensure a smooth and safe transition from hospital to home for all, ensuring everyone receives the information and support they need to help with their recovery at home and beyond.
