The team has also undergone a rebranding, transitioning from Discharge Coordinator – Hospital Discharge to Community Connectors. This reflects their evolving role in supporting both Acute and Community Hospitals across BCP, while expanding their community engagement and support to wider professionals and the public through signposting, information stations and local events.
The Virtual Hub remains a key part of our service, continuing to provide frontline health and social care professionals with essential access to voluntary sector information, guidance, and support.

We help residents across Bournemouth, Christchurch, and Poole (BCP) access the support they need to thrive. Whether it’s promoting physical and mental wellbeing, offering emotional support, or connecting people with local resources and activities, our initiatives are designed to make a real difference in people’s lives.

If you would like to learn more about how we do this, check out our Community Connector Pack.
