Access to Community Support & Services evaluation

A partnership project funded by NHS Dorset

We’re proud to celebrate our Access to Community Support and Services (ACSS) project, as we begin the end-of-year evaluation report. A collaboration with local charities and community groups, the project demonstrates the power of a shared data set in fostering stronger, healthier communities. 

We launched a Service Finder, which is currently being accessed by over 375 frontline workers across the county of Dorset. The tool enables professionals to connect people with services that address key needs such as mental health, long-term health conditions, and social isolation. 

From frontline workers

Service Finder can simplify referrals, improve efficiency, and enhance service delivery for professionals across health and social care. Between April – November 2024, 1468 searches were conducted by frontline workers. 

The dataset has grown from 150 services in November 2023 to 1289 services in November 2024, spanning a wide range of needs: 

Top service types

  • Hobbies, nature, and leisure activities: 546 
  • Community and social groups: 472 
  • Mental health and wellbeing support: 325 

Top circumstances served

  • Long-term health conditions: 241 
  • Mental health conditions: 201 
  • Older people: 191 

To maintain data accuracy, regular assurance cycles were implemented. A recent audit showed 93.7% data accuracy, reinforcing Service Finder’s reliability as a trusted resource. 

Volunteer contributions 

The project also provided meaningful opportunities for volunteers. Five volunteers were crucial in preparing data for the shared dataset. One volunteer highlighted how the role rebuilt their confidence post-COVID and enabled them to pass an administration course – a powerful testament to the project’s wider impact.

What happens next 

CAN will continue to sustain and expand the project, to help ensure that every resident can find the support they need. 

As we continue to refine and promote Service Finder and look to integrate with existing directories, we contribute to a stronger network of support across the county of Dorset—where individuals, carers, and professionals can find the right help, at the right time, and with ease.

Service Finder is just one way the shared community support and services data can be displayed. In 2025, we will embed this information into the Access Wellbeing website so that residents can find the information they need to stay well.
