Getting home sooner – Helping patients and saving money for the NHS

Imagine being stuck in the hospital even though you’re well enough to go home. Some patients find themselves in this frustrating position, but proactive interventions can help. One of the many community projects that CAN run is helping people get back home quicker and making a big difference for the NHS.

Think of it like this: CAN are able to overcome the little (or big) barriers that might be stopping someone from leaving hospital. Maybe their home needs tidying up, or they need some special equipment delivered. By connecting hospital staff, social teams and volunteers to make things happen quickly, CAN is helping people get home sooner, safely and comfortably.

Our programme also saves the NHS a lot of money. In just the last few months, CAN’s work has saved the NHS around £80,500 which can be used for other important things to help people in our community.

How CAN helps – Mr Q

Mr. Q was a patient in the hospital and ready to go home in time for Christmas, but his residence needed some work first – a good clean, rubbish taken away, and mould treatment. He couldn’t afford to have the work done himself, but without it, he was unable to go home safely.

A CAN Pathway Coordinator successfully applied for a grant and organised the required cleaning (the cleaning team even put up his Christmas tree!) Hospital staff were able to arrange delivery of the equipment he needed into a safe and clean environment. CAN arranged  daily check-in calls for Mr Q and gave him information on getting extra support.

The grant needed to get Mr Q home was £960, which saved the NHS a massive £13,040! A little bit of help can make a big difference, not just saving money but making sure people feel supported and connected as they go from hospital home.
